Facilty Use Request Form

For External Organizations Only

External organizations must use this form to request use of Eastern Florida State College facilities. Before submitting the form using the button at the bottom, be sure to fill out all applicable fields and those marked with an * are required.

All requests are subject to review, which can take up to 60 days. Submitting this form does not guarantee a reservation and some requests may be denied.

If approved, the Requestor will be notified and the terms and conditions of approval will be set forth in separate documents. Failure to follow the terms and conditions of approval may result in cancellation. Events which create a conflict with normally scheduled student activities or pose any threat to the safety of students on campus will not be approved.

Refer to the main Facilities Usage and Rental web page for more details and contacts and for information about the separate process for requests from internal college departments to reserve an EFSC facility.

Use the calendar tool to choose the start date and time for your event.
Please choose the anticipated ending day and time for your event.
Use the dropdown to choose a Campus and then complete the next field indicating what area on that campus.
List the exact building(s) and/or outdoor space(s) you are requesting.
Please be detailed and use the upload button at the end of the form to provide additional information if needed.
List any high liability or potentially dangerous activities that would be part of your event such as fireworks, extreme sports, zip-lining, etc. If there are none you can enter N/A, but you must enter some content.
If there are partner organizations, list them and include a Contact Name and Phone Number for each organization listed.
List the estimated total number of people attending overall, even if they will not all be there at one time.
If you are charging for the event, list the cost or type free.
This is only required if you are charging for the event.
Will any event revenues be shared with EFSC?
If this is a paid event, choose yes or no on the revenue sharing question.
Will food be served?*
Will alcohol be served?*
Will Event Coordinator(s) agree to provide commercial general liability insurance with minimum limits of $1,000,000.00 per occurrence naming Eastern Florida State College as an additional insured and provide that certificate no later than two weeks prior to the event?*
If the use of alcohol is being requested, will Event Coordinator(s) agree to provide additional liquor liability insurance (separate from any host liability coverage that may be available under any CGL policy) with minimum limits of $1,000,000.00 per occurrence naming Eastern Florida State College as an additional insured and provide that certificate no later than two weeks prior to the event?*
Will Event Coordinator(s) agree that all deposits and fees associated with the event be paid no later than two weeks prior to the event date?*
Examples: IT Support, projector, additional security, etc.
