Claims Process

How do I start my claim?

All incidents (accidents, injuries, property damage, thefts, violence, etc.) that occur on College property must be reported to Campus Security and/or an immediate supervisor immediately or as soon as it is safe to do so. Campus Security will generate a report and forward the report to Risk Management for review.

Employees that experience a work-related injury or illness should contact their campus security office to initiate the Worker’s Compensation Claims process

  • Avoid making any statements about what happened or making promises to an injured person at the scene
  • Fill out an Accident Incident Form (Campus Security will assist you as part of their case report)
  • Also fill out the Allied Health Professional Liability Claim Form

Education/Training Student Accident Insurance

  • Fill out an Accident Incident Form (Campus Security will assist you as part of their case report)
  • Fill out an Education/Training Student Accident Claim Form. To comply with policy provisions, this form must be submitted to Risk Management within seven (7) days of the injury in order for it to be submitted to A-G Administrators

Students are responsible for submitting all related medical bills resulting from an injury, as well as any other supporting documentation.

Example of a Claim: Student suffers a laceration requiring medical attention while participating in a clinical experience as part of his or her education or training coursework.

Intercollegiate Athletic Student Accident Insurance

Facilities Use Liability

If the College grants permission to use College facilities, it is with the express condition that the user assume full responsibility for any loss or damage resulting from such use.

Student Travel Liability

Students must complete and sign the Student Travel Assumption of Risk, Release, and Hold Harmless Agreement Waiver and send the originals to Risk Management prior to the trip taking place.

EFSC also requires detailed information about the facilities provided by EFSC for student use during the trip. This includes the name/address of any lodging facilities, room numbers, date/time of check-in and check-out, plus similar information for other spaces provided by EFSC and used exclusively by EFSC students during the trip.

Travel sponsors should use the Overnight Student Travel Log to document the required information. Upon return, the completed log must be submitted to Risk Management.
