Phlebotomy Program Accepted Student Requirements
Students who are accepted into the program will need to meet the following requirements and submit the required forms. Except for CPR and AIDS certification, all must be completed prior to start of classes, with completed Acceptance Packets due by the Tuition & Fees deadline of the current term. Detailed instructions to meet all requirements will be explained at the new student orientation and are included in the Acceptance Information Packet.
ACCEPTANCE PACKET & FORMS:
Downloadable Acceptance Packets and Forms are available each term after acceptance letters have been mailed, usually one to two weeks after each term’s application deadline.
Please note: The Acceptance Forms PDF must be downloaded and saved to your computer and then opened in Adobe Acrobat or Adobe Reader. Once you open it in Adobe, you can type your information in, digitally sign the documents, and save the file with your name as part of the file name (for example: "JaneDoe-phlebotomy-spring2019-forms"). Instructions to create your digital signature are in the packet, and also available online.
Please email the acceptance forms as an attachment using your official student Titan email account (no other email accounts accepted) to Program Manager Kristie Fuller at email@example.com. Packets will only be accepted in digital format. Please do not print or mail your packet.

BACKGROUND, DRUG SCREENING, IMMUNIZATION STATUS, & INSURANCE:
Students eligible for registration in the Phlebotomy program will be required to have a Level 2 Background Check and Drug Screening. Applicants must be drug free and free of offenses that would disqualify them from the student clinical experience in a healthcare setting. The background check will include Residence History, Fingerprinting, Nationwide Sexual Offenders Registry search and Florida Statewide Criminal Records search. The cost of the background check and drug screening is the responsibility of the student. Students are required to create an account with Castle Branch (formerly known as Certified Background or Certified Profile) and use the Phlebotomy Package Code (BA90). Please do not create this Castle Branch account until you receive an official acceptance letter. Instructions are included in the acceptance packet. Results from other background checks or drug screens will not be accepted.
CPR and AIDS:
All students of the Phlebotomy program will be required to have current American Heart Association Course Healthcare Provider CPR certification and AIDS104/BLOODBORNE PATHOGENS certification. Note: These requirements are met during the semester and included in the Phlebotomy program.
All students of the Phlebotomy Program must show proof of the following immunizations. All immunization records are to be uploaded to the student's Castle Branch account. More information on each requirement can be found in the student acceptance packets or by contacting the program manager.
- MMR 2-shot series (aka: mumps, measles, rubella)
- Tdap within 10 years (aka: Tetanus, Diphtheria, Pertussis). NOTE: this is different from TD
- PD skin test (aka: TB screening)
- Varicella (aka: chicken pox) NOTE: A history of the disease will not be accepted. A positive titer test, or proof of 2-Shot series vaccination, must be provided.
- Hepatitis B (series of three injections)
- Flu Shot (for the flu season in which the student will attend clinicals) **Fall students will have to wait until after classes start when the official flu season begins to receive their shot and provide evidence
All students who are registered in a Health Sciences Campus program are required to purchase the "Professional Liability" insurance at a cost of $16.00 per year and "Accident Insurance" at a cost of $10.50 per year (Aug.-Aug.) through EFSC. This charge should be automatically applied to the student’s account as part of tuition and fees.
